Inform us if you change your name and address or telephone number. If you move out of the practice area, you will be required to register with a more local practice.

How To Register as a Patient
If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records.
To register, call into the surgery bringing with you:
- A completed General Medical Services (GMS1) form.
- Two forms of identification (ID), with one being in the form of photographic ID and the second being a proof of address.
Health Check
An appointment for a health check performed by the nurse is offered to all new patients; an appointment can be made at reception.
Temporary Residents
The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.
To register you will need to bring your passport and complete a GMS3 Temporary Resident Registration form which is also available from reception.
